Youth Ministry Registration Forms

For Families with Both Children and Youth

In order that our religious education department can better serve the formation needs of our young parishioners, we ask that parents register them for formation with the religious education department. This can be accomplished by filling out all of the required registration forms ( see below and refer to the correct section for the children's ages) and submitting them to the religious education office in one of several ways:

1. Bring the forms to the parish office.  During business hours, turn them in to a member of the parish religious education staff, either in the main administrative offices or in the Youth Ministry office.. Outside of business hours, deliver them via the mail slot in the parish office door off of the main hallway of the Family Life Center or the "lock box" at the end of the Family Life Center hallway near the Youth Ministry office.
2. Mail the completed forms to the parish office .
3. You may fax the completed forms to the parish fax machine (phone number at this page).
4. You may complete the forms, scan them in, and email the files to this email address:
   sally@smlj.org for Children's Faith Formation or janet@smlj.org for Youth Ministry.  Either address will work for forms covering both groups (K thru 6th grade AND 7th thru 12th grade).
5. Finally you may download or get original forms, fill them out, and place them into the collection basket at Mass.

Registration is only considered complete if the required funds for youth classes are submitted along with or in followup with the required forms.

Forms can be picked up from the parish religious education or Youth Ministry offices ( during these days & hours ) , or downloaded from this website (see the sections below) to fill out and submit to the office. Refer to the sections below about which forms are needed for your children/youth.

The forms included in this packet are required to be filled out in order to have a qualified registration for Children's Faith Formation. Included in this packet to be filled out for all students who will be in Grades Kindergarten thru 6th in 2018-2019 are:

• Religious Education Registration Form for 2018-2019 (2 pgs.)
• Liability Release Form for 2018-2019 (1 pg.) (one required for each family with elementary-age children)
• Faith Formation Calendar for 2018-2019 (1 pg.)

Also included in this packet are required forms specific to those students (generally 2nd grade in the upcoming school year) who will be receiving formation for First Reconciliation and First Eucharist:

• Sacrament Registration Form for 2018-2019 (1 pg.)
• Sacrament Notice Sheet (1 pg.)

Registration is only considered complete if the required funds for Children's Faith Formation are submitted along with or in followup with the required forms.

The following TWO forms are required to be filled out in order to have a qualified registration for Youth Ministry:

• Registration Form for 2018-2019 (2 pgs.)
• Youth Ministry Liability Form (2 pgs.) (one required for each family)

Registration is only considered complete if the required funds for Youth Ministry classes are submitted along with or in followup with the required forms.

In addition, off-site activities will need a specific release form for each child for each activity. See the Youth Ministry website.

The Registration Form is common for both Children's Faith Formation (formerly C.C.E.) and Youth Ministry.  One form should suffice for all children of a family.

The Children's Faith Formation Liability Release Form is only for children of K through 6th grade.  A single form is required for each family with elementary-age children.

Also for children in Children's Faith Formation, one Parent Responsibilities form is required and will cover all of the K through 6th grade children in each family.

The Youth Ministry Liability Form is only for youth from 7th grade through 12th grade.  A single form is required for each family with 7th grade through 12th grade youth.

Funds for the entire family can be noted on the single Registration Form and paid via a single check.
